Antidiuretic Hormone (ADH), also known as arginine vasopressin (AVP), is a key hormone synthesized in the hypothalamus and stored/released by the posterior pituitary gland. Its primary function is to regulate the amount of water excreted by the kidneys. When ADH levels are high, the kidneys conserve water, concentrating the urine. When ADH levels are low, the kidneys excrete more water, diluting the urine. This process is critical for maintaining overall body fluid balance, blood pressure, and plasma osmolality.

Clinical Significance:

  • Diagnosing Diabetes Insipidus (DI): The ADH assay is essential for distinguishing between different types of diabetes insipidus. Central DI occurs when the brain doesn’t produce enough ADH, while Nephrogenic DI occurs when the kidneys don’t respond to ADH properly.
  • Identifying SIADH: Syndrome of Inappropriate Antidiuretic Hormone secretion (SIADH) is a complex condition where the body releases too much ADH, leading to excessive water retention and low blood sodium (hyponatremia). ADH testing is a key component of the SIADH workup.
  • Investigating Water & Electrolyte Imbalances: ADH testing helps clarify the cause of unexplained hyponatremia (low sodium) or polyuria (excessive urination).
