The Culture & Sensitivity (C&S) Aerobic Bacteria, High Vaginal Swab (HVS) test is a critical diagnostic tool used to identify pathogenic aerobic (oxygen-requiring) bacteria present in the upper vaginal vault and cervix. The test consists of two primary steps: Culture, where the swab sample is introduced to growth media to isolate and grow any existing bacteria, and Sensitivity, where any identified pathogens are tested against various antibiotics to determine the most effective treatment.

Target Bacteria

This test specifically targets common and significant aerobic pathogens that cause vaginal infections (aerobic vaginitis) or pose risks during pregnancy, such as:

  • Streptococcus agalactiae (Group B Streptococcus – GBS) – crucial for prenatal screening.

  • Staphylococcus aureus

  • Escherichia coli (E. coli)

  • Klebsiella species

  • Enterococcus species

(Note: This test does not typically focus on anaerobic bacteria, yeasts (Candida), or specialized pathogens like Trichomonas vaginalis, which require different testing methods or media.)

How the Sample is Collected

A High Vaginal Swab collection is typically performed by a healthcare provider during a pelvic exam. A sterile swab is inserted into the vagina and rotated gently against the high vaginal vault and cervix to collect secretions and cells. The swab is immediately placed into a transport medium to preserve the viability of the bacteria during transit to the laboratory.

Patient Preparation

To ensure accurate results, patients may be advised to:

  • Avoid douching for at least 24 hours before the test.

  • Refrain from using vaginal medications, suppositories, or creams for 48 hours.

  • Notify the lab if they have recently taken or are currently taking antibiotics, as this can affect bacterial growth.

Clinical Utility

This test is essential for:

  • Diagnosing the cause of persistent abnormal vaginal discharge, itching, or inflammation.

  • Differentiating aerobic vaginitis from other conditions like bacterial vaginosis or yeast infections.

  • Screening pregnant women (typically at 36-37 weeks) for Group B Streptococcus to determine the need for intrapartum antibiotic prophylaxis to protect the newborn.

  • Guiding antibiotic therapy by identifying the specific drug sensitivities of the isolated pathogen.

Turnaround Time (TAT)

The turnaround time is typically 48 to 72 hours. The culture process requires an initial growth period (24-48 hours), followed by subsequent sensitivity testing once a pathogen is isolated.
