The Bence Jones Proteins Urine Spot Test is a critical screening tool used to detect monoclonal free light chains (Bence Jones proteins) in a random urine sample. This test is typically ordered as part of the diagnostic workup for patients suspected of having Multiple Myeloma or other plasma cell proliferative disorders.
What is the Bence Jones Proteins Test?
Bence Jones proteins are monoclonal free light chains produced abnormally by malignant plasma cells. Unlike normal proteins, Bence Jones proteins are small enough to be filtered by the kidneys and excreted in the urine. Their presence is a hallmark diagnostic indicator for Multiple Myeloma, and is also found in other conditions like Waldenström’s macroglobulinemia and monoclonal gammopathy of undetermined significance (MGUS).
This “spot test” requires only a random, single urine collection and provides a rapid qualitative (positive/negative) result to help doctors determine if further, more complex testing (like urine protein electrophoresis) is necessary.
Why is This Test Ordered?
Your physician may recommend this test if you exhibit symptoms or have other laboratory findings suggestive of plasma cell disorders, including:
- Unexplained bone pain (especially in the back or ribs).
- Anemia or low blood cell counts.
- High blood calcium levels (hypercalcemia).
- Unexplained kidney insufficiency or failure.
- Recurrent infections.
- Elevated total protein levels in blood tests.
Test Methodology
The screening test is commonly performed using the Heat Precipitation Method (Sulfosalicylic Acid or SSA test). Bence Jones proteins have unique thermal properties: they precipitate (become cloudy) when urine is heated to 40-60°C and redissolve when heated further to 100°C. Note: For confirmation, abnormal results are typically followed up with Urine Protein Electrophoresis (UPEP) and Immunofixation.
