Aldolase Blood Test

Aldolase is an essential enzyme involved in glycolysis—the process of breaking down sugars to produce energy. While present in all cells of the body, it is found in the highest concentrations in skeletal muscle, the liver, and the brain. When these tissues are damaged, the cells rupture and release aldolase into the bloodstream, causing serum levels to rise.

Clinical Significance:

• Muscle Diseases (Myopathies): The primary use of the aldolase test is to evaluate muscle damage. Very high levels are often seen in progressive muscle conditions like muscular dystrophy, as well as inflammatory muscle diseases such as polymyositis and dermatomyositis.
• Complementary to CK: While Creatine Kinase (CK) is generally the first-line and most specific test for muscle damage, aldolase is highly valuable in rheumatological workups. In some patients with inflammatory myopathies, aldolase levels may be elevated even when CK levels are normal.
• Liver Disease Evaluation: Because aldolase is also present in the liver, elevated levels can sometimes indicate liver damage, such as from hepatitis. However, more specific liver panels (like ALT and AST) are typically preferred for this purpose.

When is this test recommended?

Healthcare providers typically order an Aldolase test when a patient presents with symptoms of muscle breakdown or weakness, including:

• Progressive muscle weakness, especially in the shoulders, hips, or thighs.
• Deep muscle aching or pain.
• Difficulty performing physical tasks like climbing stairs, getting up from a chair, or lifting objects.
• To monitor the effectiveness of treatments for known muscular or autoimmune diseases.

Sample Requirements:

• Specimen: Blood (Serum).
• Container: A standard Red-top tube (no additives) or a Gold-top Serum Separator Tube (SST).

Patient Preparation:

• Fasting: Patients are typically required to fast (no food or drink other than water) for 8 to 12 hours prior to the blood draw.
• Exercise Restriction: Strenuous physical activity can cause a temporary spike in aldolase levels. Patients are usually advised to avoid intense exercise for at least 48 hours before the test to prevent a false-positive result.
